REGISTRATION INFORMATION Fall 2010
Registration for fall is now CLOSED!

U6 girls
U12 boys
U14 boys/girls
New U5 program Fall/Spring 2010
*U5 players must be 4 years old by the start of first game to play*  


Practices are once week for U8 -U19.

How do I Register? Use the form from the above link, or, for players and volunteers please go to www.eayso.org. If you have not made an account there please make one. If you enter the same information we have on file, you should be able to find your kids. 
Please PRINT THE FORM, not the screen. There has to be a place to sign or we cannot accept it.

 Where does the registration fee go? AYSO Region 605 pays over $2,000 per week to reserve and use Mallard Creek Park, for our practices and games. We purchace Goals, goal nets, corner flags, field paint and paint machines. Uniforms are also provided for players and certified referees, and we pay for training for all the coaches and referees.  We purchase trophies for your child as well as first aid supplies. Lastly, $14.75 of each childs registration fee goes to the AYSO National Support and Training Center (NSTC) for supplemental insurance and the outstanding support they provide.

 Players are placed in divisions based on date of birth.  Your child's division can be determined from the following table:

Refund Policy

Players withdrawing prior to August 20th, 2010 will be issued a full refund. After August 20st refunds will NOT be issued except for the following reasons: 

Registered player for the present season is injured and unable to play. (A Dr.’s note is required)    OR

Registered player has moved out of the area.

The cost of items not returned (including uniforms) will be deducted from your refund.

 Request a refund for your child and include your address, division for your child and reason for refund.  Refunds take about 3 weeks to process.

Late Registration

Late registration begins after the last regular registration event, All late registration applications will be on a wait-list basis only and your check will be deposited only if the child is registered. Contact the registrar to see if there are any openings. If you don’t know your child’s division, check the age chart above.
